Please follow the steps below to ensure your case is submitted correctly:

STEP 1 - Place your order 

Begin by submitting your order using the following link:

STEP 2 - Confirm your order via email

Once the order is placed, you will receive a confirmation email with your order details.

Please check your Spam or Junk folder if you do not see the confirmation in your inbox.

If you still haven’t received it, it's possible that there was a typo in your email address during submission.

You may need to resubmit the form with the correct email.

STEP 3 — Prepare and Upload the Case Files

After receiving your order confirmation, you may proceed to gather and upload the required digital files:

Required Files:

CBCT files in .DCM format

Digital impressions in .STL format

These files must be placed together in a single ZIP folder.

Please name the ZIP folder using the Patient ID you specified during the order.

Do not use full patient names to protect patient confidentiality.

📌 Example Patient ID Format:

If the patient’s name is Jocelyn Richardson and her case relates to teeth 8, 9, and 10, you might use:

JR5668-teeth-8-9-10

In the order form, use JR5668 as the Patient ID.

So when you create the ORDER ONLINE the patient ID name should be  JR5668. (instead of Jocelyn Richardson) This is to protect the patient's confidentiality because we have to send emails back and forth.

STEP 4 — Upload the ZIP Folder

Upload your ZIP file at the following link:

https://cloud.ceraroot.com/

Important Notes:

• Maximum file size: 1 GB
  
  

• On the upload form, you must enter:
  
  

  • The Order Number you received in the confirmation email
    
    

  • The exact same Patient ID you used when placing the order
